Goal-Based Accountability

Regular check-ins and progress reviews tied to the participant's individual development plan and milestones.

Case Coordination

A single point of coordination connecting participants with employers, housing providers, treatment providers, and government agencies.

Support-Group Referrals

Referrals to appropriate peer support groups including 12-step programs, SMART Recovery, and other community-based options.

Counseling Referrals

Connections to licensed professional counselors for mental health, trauma, and personal development support.

Substance-Use Treatment Referrals

Referrals to licensed and accredited substance-use treatment providers and programs.

Mental-Health Provider Referrals

Connections to licensed psychiatrists, therapists, and mental health clinics.

Medical Provider Referrals

Referrals to primary care physicians, clinics, and specialists for physical health needs.

Anger-Management Referrals

Connections to certified anger-management programs and providers.

Parenting Education

Access to parenting classes and resources supporting healthy family relationships.

Family-Reunification Support

Coordination and support for participants working toward reunification with children and family members.

Faith-Based Support

Connections to faith-based programs and communities when requested by the participant.

Transportation Coordination

Assistance with transportation to appointments, treatment sessions, support groups, and other required services.

Documentation & Appointment Tracking

Support maintaining identification documents, appointment schedules, and compliance records.
